Insurance and Rates
We currently accept various insurance policies and self-pay. We will be considered “out of network” if we do not participate with your insurance plan. If you wish to be reimbursed for your sessions, you will need to consult your insurance company to determine their policies regarding mental health benefits for out-of-network providers. We will provide you with a “super bill” that can be submitted to your insurance company for the possibility of reimbursement from your insurance provider. In Colorado, we can accept all insurances relying on their out-of-network benefits.

Comprehensive Psychiatric Evaluation
This is the first visit with your nurse practitioner.

This will be all subsequent visits after your first visit with your nurse practitioner.​

Lab Testing

Lab testing will be discussed in session and may be recommended depending on your treatment goals.

Routine labs may be covered by your insurance, when not covered prices vary greatly.
Forms

Forms may be provided, at the clinician's discretion.

Forms will be filled out only for established clients.

We do not offer long term disability certification.

Short-term disability may be considered for established clients whose treatment requires time off work. This will be at the clinician's discretion.

Disability paperwork: $75 per letter or form

I do not provide disability evaluations, worker's compensation evaluations, forensic evaluations, or provide legal services or testimony. Should you require legal testimony at some point during our treatment together, you will need to retain an independent forensic psychiatrist.

Phone Calls

Phone calls with family members or other providers such as therapists or primary care doctors$25 if greater than 15 minutes.

All client phone conversations running over 15 minutes will be charged: $145 (the price for a 30-minute follow-up appointment)

Difficult Prescriptions

Difficult prescriptions/prior authorizations (PA’s taking more than 15 minutes): $25.00

No Surprises Act
In compliance with the "No Surprises Act," Butterfly & Blossom Psychiatry provides "Good Faith Estimates" upon request, and these will be sent to all patients within 3 business days.
